Could anyone give me a quick tutorial in making a form I have designed activate for on-line use?

The form will be sent to people who will fill in the appropriate information and return it to me. Is this possible in Quark?

    Many thanks.

Will everyone you send the form to have Quark? If not, you may want to investigate the possibilities of saving your Quark file as a PDF. All will be able to read the PDF, and some will be able to fill it in via the computer, but some will have to print it out and send it to you another way. It all depends on what software they have (full Acrobat, free Reader, etc.).

I was thinking of saving the Quark form as a PDF but didn't (and still dont) know the restrictions/capabilities.

    Many thanks.

See if Adobe has a trial version of the full version of Acrobat. There are a few different flavors (different feature sets for different prices) to choose from. You would have to typeset your form in Quark, convert it to a PDF and then open it in Acrobat. From there you could use the "forms" features in Acrobat to add fields that the user could fill in.

    What migman said,

    or

Make a web layout in quark, add all fields using smilar tools to those offered in Adobe's acrobat and export html.

    Either way,

the cgi programming will still have to be done to make the form truly interactive. If you have an IT department that handles your website, coordinate with them. If not, you can probably find some to do the work freelance.

Instead of finding someone in IT, you can get a ready-made form-mail script from your web hosting provider (most offer a pre-made one with instructions for use), and you can do this yourself. The server's form mail script can be CGI, ASP, PHP, JSP, Perl, Python, etc. Isn't there some documentation about all this in the Quark manual?
