How do I create a hot spot?

Seriously though, and if you pardon my ignorance, what is a hot spot?

My understanding is that web options were only introduced in version 5 upwords. Even then though you need to specify in version 5 whether you want a print version or web version and switching between the two was not possible. Unsure whether they have changed this in version 6, perhaps someone else could verify this. An alternative if you need to work in v4.11 would be to create a PDF and use acrobats capabilities to make the links? I know not the best way but jsut a thought?!
